They cited "operator error" for the incident. Reason being they say the operator should NOT have allowed him on
The problem was the war vet had no legs he lost both in the war and because his balance as a human body was off .it pulled him right out..
Technically these rides are made for people with arms and legs, thats how it is designed..

This wasn't the first time someone got hurt on the superman, in 99 close to when it opened a heavy person fell out and got hurt. If i remember correct they installed the seat belts after and if you cant get it to latch you cant ride.
