1.My first season when I caught my heel edge and slammed my back and head (no helmet) down the fall line.
2. Two weeks ago will probably be memorable. I just sat on the chairlift and my bindings got caught under. The chair just churned me under it and dragged me forward like a roadkill.

I try to keep every season pass I have ever owned. Earliest ones are from the early 90s. Now I have a few from other states and the collection continues to grow.
Oh and trail maps. At one point I had a mural of sorts with every trail map of every mountain I've ridden... but I kept going back to some places and the maps kept changing, so it was hard to keep it going. Whole thing was about 7 feet tall and maybe 15 feet wide. When I redo it, I'll cut off all the extra crap (captions, legends) so that it is only maps.
