As far as the workout part, I am probably in the best shape that I have been in years. I try to wakeboard twice a week, and that sport is a damn great workout. I bet this coming season I am going to be in great shape to last much longer days than some seasons ago.
As far as the food goes; chocolate, beer and sushi is the best diet to stay in shape .

Diet - veggies (not potatoes, the non starchy ones) every meal, lean protein source each mean, eat as much fruit as you want, eat whole wheat breads, dont drink your calories
Workout - talk to a trainer locally that can help build something that suits your goals. if possible find a sports specific training facility and not an average gym. or take a look at something like crossfit.com and their bodyweight WODs. |

Try following the Body For Life model written by Bill Phillips.
I have used it in the past and had great results and will be starting it again on Tuesday. The last time I followed it I dropped 30 pounds in 3 months and put on some decent muscle. It helps you plan your workout and your meals. No "dieting" since you get to eat so much food you will never be hungry. |
